Budesonide

Budesonide Nasal Spray
  • 24-hour allergy relief

Budesonide nasal spray is a metered-dose, manual pump spray unit containing a suspension of budesonide.

Each metered-dose pump actuation of budesonide nasal spray delivers 64 micrograms of budesonide.



Indications and Usage
Budesonide nasal spray is a corticosteroid indicated for:
  • Treatment of seasonal (hay fever) and perennial (year-round) allergic rhinitis
  • Treatment of Nasal polyps
  • Prevention of new nasal polyps from appearing after surgery (polypectomy)
Dosage and Administration

  • Treatment of Allergic Rhinitis:
Adults and children 6 years of age and older: 
The recommended starting dose is 256 micrograms daily.
The dose can be administered once daily in the morning or divided into two administrations, morning and evening. This implies 128 micrograms (2 x 64 micrograms) into each nostril in the morning or 64 micrograms into each nostril morning and evening. 
When the desired clinical effect is obtained, the maintenance dose of 32 micrograms in each nostril in the morning may be sufficient for some patients.
Symptomatic relief occurs in some patients within only 5-7 hours after the start of treatment. The full effect is only obtained after a few days of treatment. 
Treatment of seasonal rhinitis should therefore, if possible, start before exposure to the allergens.

  • Treatment or prevention of nasal polyps
The recommended dose is 256 micrograms daily.
The dose can be administered once daily in the morning or divided into two administrations morning and evening. After the desired clinical effect is obtained the maintenance dose should be tapered to the smallest necessary to control the symptoms.
Note: If Budesonide prescribed for hay fever, patients should start using it 2-3 weeks before the hay fever season starts to feel the full effect.

Contraindications
  • Budesonide nasal spray should not be used in CHILDREN younger than 6 years old.
  • Concomitant treatment with ketoconazole or other potent CYP3A4 inhibitors should be avoided. If this is not possible, the interval between administrations of the medications should be as long as possible.
Pregnancy and Breastfeeding
FDA pregnancy category B. It is not known whether budesonide passes into breast-milk.

Mechanism of action
In the treatment of nasal symptoms, the primary action of nasally applied corticosteroids is anti-inflammatory. Budesonide inhibits the IgE and mast cell-mediated early- phase allergic reaction. It also inhibits migration of inflammatory cells into the nasal tissue (the late-phase or late onset allergic reaction), which may play a significant role in the pathology of chronic rhinitis.
During the late phase allergic reaction, eosinophils, neutrophils, basophils and mononuclear cells produce inflammatory mediators which cause a reappearance of nasal symptoms.
